Job Title: Day Habilitation Manager

Immediate Supervisor: Program Coordinator
Hours of Work: Full time, 40 hours per week
Weekend Work: Special pre-planned events only
High school diploma or GED is required. At least one year experience working with people who have a developmental disability.  Minimum 1 year supervisory experience. Valid driver’s license required, must meet criteria for infractions and points.
General Health:  Must be able to perform the following:

  • Manage daily operations of day program site.
  • Develop ISP goals for assigned program participants per ELP planning.
  • Attend ELP meetings and team meetings for assigned program participants.
  • Train staff on implementation and documentation of ISP goals.
  • Assure that ISP goals are documented daily by staff for each participant.
  • Supervise and approve daily activity and assignment schedule, including van runs that are prepared by Day Hab Supervisor.
  • Train and support staff as necessary; encourage feedback and respond to questions from staff.
  • Schedule and conduct monthly staff meetings.
  • Delegate responsibilities when appropriate.
  • Ensure that participants are treated fairly.
  • Work in conjunction with Day Habilitation Supervisor to develop and carry out activities for program participants.
  • Ensure that daily activities and supports are carried out by staff as assigned.
  • Be knowledgeable of policies and procedures and ensure that they are followed by all staff.
  • Provide individual support per needs.
  • Work shifts as part of minimum coverage instead of scheduling staff for overtime shifts.
  • Implement person-centered philosophy in accordance with Agency values.
  • Purchase necessary items as needed and approved by Coordinator. File all receipts.
  • Reconcile expenses as required for financial reporting.
  • Complete documentation of services, including attendance and monthly summaries as needed.
  • Maintain communication with supervisor and nurse for any medical concerns.
  • Ensure compliance with all dietary orders and meal protocols.
  • Maintain familiarity with behavior plans per individual and ensure all staff follow behavior plans.
  • Follow and enforce universal precautions
  • Maintain medication administration certification and pass medications as prescribed.
  • Update at least yearly all necessary Therap documentation, including IPOP’s, IDF’s and EDF’s.
  • Follow all agency, state and federal regulations to maintain programmatic compliance.
  • Report all incidents, including allegations of abuse and/or neglect to supervisor immediately. Complete or assure completion of GER’s for all reportable incidents.
  • Attend and successfully complete all training per agency, state and federal requirements.
  • Assist with scheduling staff for mandatory trainings.
  • Maintain established staffing ratio.
  • Interview and make recommendations for hiring staff as needed.
  • Follow procedures for approving and using PTO and calling out. Counsel staff not following procedures.
  • Serve as a positive role model for program participants, co-workers and community members.
  • Abide by agency dress code and report to supervisor employees who are not compliant.
  • Assure program area is neat and sanitary. Oversee cleaning responsibilities on schedule. Order paper supplies and cleaning supplies as necessary.
  • Use a positive approach when handling difficult situations. Maintain professionalism.
  • Report safety hazards immediately to supervisor.
  • Ensure exits are clear and unobstructed.
  • Complete safety checks on vehicles, first aid kits, fire extinguishers and medication cabinet as assigned by supervisor.
  • Advocate for participants in the day program.
  • Be knowledgeable of participant’s desires, preferences and valued outcomes. Communicate this information to supervisor as needed.
  • Maintain familiarity with 21 Personal Outcomes and support participants to achieve their outcomes.
  • Suggest changes or services that can assist participants with achieving outcomes.
  • Ask participants if they are satisfied with their services and respond to all concerns and suggestions.
  • Concerns that cannot be handled immediately should be brought to the attention of the Program Coordinator.
  • Other related duties as assigned by supervisor.
