Licensed Practical Nurse
Location: Day Supports / Residential / Offsite                                         
FLSA: non-exempt
Reports To:  Supervising RN                                                          
Physical Requirements: General good health.  Required to sit at computer for extended periods of time
Hours of Work: Full time – approximately 37.5 hours per week.  Monday – Friday 8:30a – 4:30p.  Occasional evening / weekend hours may be required.
Job Qualifications:
  • Must be currently licensed and registered as an LPN by the NYS Education Department. 
  • Experience in a clinical nursing setting and/or experience with the developmentally disabled preferred.
  • Valid NYS driver’s license
  • Travel is required
Major Responsibilities:
  • Assist the RN in directing nursing care
  • Report any concerns or medical problems to the RN
  • Assist and complete medical documentation in a timely and accurate manner
  • Assist with medications and treatments – such as medication administration, wound care, g-tubes etc.
  • Assist with medical appointments
  • Monitor medical supplies and medications
  • Work closely with the RN following the care within the LPN scope of nursing
  • Provide medical services to individuals (following the LPN scope of nursing), in compliance with agency, local, state and federal regulations.
  • Attend mandated training as required to ensure LPN license is current
