Location: New Dimensions in Health Care - Amsterdam, NY                                  
FLSA: Non-Exempt
Reports To: Assistant Director of NDHC                                           
Physical Requirements: Meet all NYSDOH Pre-Employment and annual health requirements
Hours of Work: Full Time, Monday – Friday 40 hrs. / Week
Pay Range: The pay range is $20.30 to $29.03


Job Summary:
The LPN provides direct assistance to the NP/Physician in preparation of clinical/medical services.  Working in collaboration with the medical staff, the LPN prepares documentation as it applies to direct patient care. The LPN helps to ensure compliance and quality control as it applies to the daily operations of the clinic through equipment checks and supply inventory within the scope of guidelines set by the DOH and CDC.

Job Qualifications:
  1. A Licensed Practical Nurse, currently registered by NYS Dept. of Ed.
  2. Minimum of 2 years’ experience in a progressive health environment (hospital, regulatory agency, or outpatient setting involving medical services.
  3. Working with people with developmental disabilities preferred.
  4. Completion of phlebotomy training program or phlebotomy experience preferred.
  5. Must have excellent clinical, interpersonal, and organizational skills.
  6. Must be able to read, write and speak the English language.

Major Responsibilities:
  1. Provide direct assistance to Physicians/Physician Extenders with the preparation of clinic medical services
  2. Prepare medical records prior to the clinic at least 24 hrs. in advance of each medical clinic. This shall include reviewing the previous appointment and tests ordered to ensure the information ordered is available for physician review and action at the time of the clinic.
  3. Work within the scope of practice of a Licensed Practical Nurse and under the direction of an NP and/or physician. 
  4. Participate in completing necessary paperwork needed to submit for prior approvals that involve pharmaceuticals, equipment, nutritional supplements, etc.
  5. Document services performed in the medical record in accordance with Health Center Policy and Guidelines.
  6. Provide patient follow up and continuity of care services as requested by the physician such as arranging follow up, consultations and referrals with other providers; laboratory and other testing, patient instructions, and as directed.
  7. Assure instruments and supplies used in the performance of patient related services are cleaned, stored and maintained in accordance with the manufacturer’s recommendations and in compliance with NYS regulations for Infection Control, and OSHA regulations.
  8. Conduct compliance checks and quality control for all identified equipment and services logs as outlined in policy and guidelines, such as checking refrigerators for temperature checks, supplies and medications for expiration dates, etc.
  9. Order supplies on at least a monthly basis and as needed to meet the needs of the clinic.
  10. Follow Universal Precautions and the New Dimensions in Health Care Blood Borne Pathogens plan. This position is classified as a Category I for the potential for exposure.
  11. Participation as indicated in CQI process, infection control program, OSHA and DOH compliance and other like processes as needed.
  12. Understand and implement the principles of PCMH
